As an EU citizen you are entitled to the free movement within the EU
and to enter and stay on the territory of the Czech Republic you need
only your passport or ID.
If you intend to stay on the territory of the Czech Republic for
longer then 30 days you are supposed to proceed with those steps:
1. After you arrival you are obliged within 30 days to report the
place of your residence in the Czech Republic to the Foreign Police.
2. You are entitled to apply for a temporary residence permit (if the
period of your stay will exceed 3 months). The application should be
submitted on the territory of the Czech Republic to the office of the
Ministry of the Interior.

of stay on the territory.
Regarding your concrete questions:
1. The appropriate institution to be dealt with in the matter of the
administration of residence permit is the Ministry of the Interior.
And partially (regarding the obligation to inform about a place of
stay on the territory) also the Foreign Police.
2. necessary document for application for temporary residence permit
are the following:
a) travel document or ID
b) document confirming the purpose of your residence (in your case
document confirming your employment in the Czech Republic)
c) photographs
d) a document on medical insurance (this provision does not apply if
the purpose of the residence is employment)
e) a document proving that you have accommodation in the Czech Republic.
3. You have to present the above mentioned documents to the office of
the Ministry of the Interior when applying for the temporary residence
permit.
4. As an EU citizen you do not need a work permit. Your employer is
only obliged to inform the Labour office about your employment.
5.+ 6. Yes, under general conditions the employer is responsible for
paying the medical insurance, which is deducted from your salary. The
coverage of the insurance is the same as for the Czech citizens 7.
Yes, you have to prove that you have accommodation in the Czech
Republic.
